Purpose of Classification
The purpose of positions in this classification is to advocate on behalf of patients; assess patient needs, and develop care/treatment plans and discharge plans in an acute care psychiatric hospital. The work is performed under the supervision of the Client Services Manager.
Essential Duties and Responsibilities
Serve as liaison with consumers/clients, families, referral sources, community agents, third party payors and others regarding services as needed; obtain and provide accurate and timely reports of consumer/client status, progress and needs.
Work as part of interdisciplinary team in developing and implementing treatment plans that provide appropriate mental health services to clients.
Obtain social history information from the client, the family, or other agencies as part of the evaluation process.
Provide therapy to individuals and families in individual and group modalities of psychotherapy.
Assist inpatient clients with discharge planning, including living arrangements, outpatient treatment referrals to various agencies and/or community resources.
Act as liaison with staff from contracted counties regarding their clients who are receiving inpatient services.
Compile collateral information such as treatments, prescriptions, diagnoses, social and other data. Act as liaison to Psychiatrist, Corporation Counsel regarding patient legal status and family members.
Explain the nature of probable cause hearings and final hearings to clients and/or families.Also, arrange court examiners for final hearings.
Complete and maintain necessary documentation to applicable requirements, codes and policies.
Responsible to know and practice the Safety policies of the County.Perform all job tasks in a safe and prescribed manner.
Additional Tasks and Responsibilities
Initiate guardianships and protective placements as needed for patients that are on the Admissions Unit.
Arrange for appropriate transfer of out-of-state clients and requests financial reimbursement for units of service provided due to interstate compacts on mental health
Arrange transfers of adolescents to adolescent units when necessary.
Report suspected elder abuse and neglect cases without the consent of clients to the Adult Protective Services Department.
Report suspected child abuse and neglect cases without the consent of clients to the Child Protective Services Department.
Attend and participate in staffings, in-services and outside agency training sessions.
Assist patient in completing applications for Medicaid, disability benefits and eligibility for indigent drug program.
Master’s Degree in Social Work, Counseling or related field is required; one to two years social work experience preferably in an acute care psychiatric hospital, or any combination of education and experience that provides equivalent knowledge, skills and abilities. Licensed Social Worker, must be licensed as an Advanced Practice Social Worker (APSW) or Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor Training License (LPC-IT), or LPC required.
