Essential Duties and Responsibilities
Identify the discharge needs of patients on the hospital unit along with members of the treatment team at the time of admission, and problem solving those needs prior to discharge.
Discharge planning, including phones calls for referrals and collateral information for discharge plan, enrolling for insurance and medication prior authorizations.
Utilization review of hospital stay and instructing inpatient and outpatient team on needed changes in status due to un-necessary restrictive environment.
Make insurance calls to complete prior authorization for hospital stay coverage, provide clinical documentation and assure payment from insurance company.
Assist with court hearings on inpatients and patients that are Wood County’s responsibility at other facilities.
Additional Tasks and Responsibilities
Utilize quality improvement outcomes in short and long range planning and in strategic planning for the facility.
Minimum of a Bachelor’s Degree in Social Work, Nursing or related field is required. Two to five years’ experience in hospital care, discharge planning and utilization review. Must possess a good working understanding of medical and mental health issues, concerns and diagnoses. Ability to assess treatment needs and assess services provided to the patient in the inpatient environment as necessary and appropriate according to law, regulations and policies. An excellent working knowledge of treatment, medications and course of hospitalizations as they apply to mental health illness. Strong data analysis, statistics and project management skills are desired. This position may require travel to and from off-campus sites. Individual may use personal vehicle or other means of reliable transportation.
