Birth to Three Program Blended Coordinator (Service Coordinator & Teacher)
Location:River Block Building - WR
FT/PT:Full-time 38.75hrs/week - WRS Eligible

Job Responsibilities:
Note:  Position works out of both the Wisconsin Rapids and Marshfield Human Services offices.

The Blended Coordinator applies Wisconsin Birth to Three Program guiding principles and implements primary coach approach to teaming in natural environments to support families of children under the age of three with developmental delays.

No supervisory responsibilities.



1. Serve as a blended coordinator by being both a Service Coordinator and a Teacher in the Birth to Three Program.

2. Demonstrate and embrace an understanding of the primary coach approach to teaming in natural environments and serve as an advocate for the effectiveness of this approach.

3. Be an active team member who participates in all scheduled team meetings and staff development opportunities/trainings.

4. Establish and maintain effective relationships with young children, parents, co-workers, the general public, and other agencies.

5. Complete comprehensive evaluations and assessments on the overall development of children by utilizing the Battelle Developmental Inventory, 2nd Edition (or other standardized evaluations/assessments as assigned), on all children who are in need of an evaluation to help determine eligibility into the Birth to Three Program as well as those who may need an updated one who are already enrolled.

6. Communicate clearly and effectively in writing when preparing written reports for child evaluations/assessments, Individualized Family Service Plans (IFSP), home visit summary/ongoing assessment notes, case notes in The Clinic Manager (TCM) software, progress reviews, transitions, and discharge including information on client strengths and deficits, outcomes/goals, recommendations, and educational plans.

7. Identify family concerns, individual family priorities, activity settings, routines, and unique characteristics when identified as the primary coach for a child/family for the purpose of incorporating the families’ desires and goals into intervention/-coaching strategies. Engage children and families in interest based family and community learning activities.

8. Conduct home visits in a variety of natural learning environments to coach overall skill development with the utilization of the resources within those environments to achieve the identified outcomes.

9. Serve as a secondary coach when needed to support other primary coaches, parents, and other care providers directly related to the IFSP outcomes.

10. Work collaboratively on various levels with Birth to Three team, parents, day care providers, and other caregivers to identify and develop strategies to accomplish identified outcomes.

11. Complete required program reports in accordance with agency and statutory guidelines within agency time frames.

12. Incorporate into daily practice the Birth to Three Program mandates and best practices, as well as Individuals with Disabilities Education Act (IDEA) Part C and how early intervention services are to be provided requiring the use of evidence-base practices that happen in real-life settings.

13. Perform all duties in compliance with standards set out in Medicaid Targeted Case Management regulations.


1. Actively participate in development and conducting of quality improvement projects.

2. Provide consultation and education services to clients, organizations, and community agencies as requested.

3. Attend in-service trainings as appropriate to meet licensing requirements, and to continue personal and professional growth.

4. Any other duties as assigned.
Job Qualifications:

1. A Bachelor’s Degree in Special Education, including Early Childhood Exceptional Educational Needs (EC-EEN) Department of Public Instruction (DPI) certification #808 is preferred. Provisional certification may be considered.

2. At least one year of experience utilizing your early childhood exceptional educational needs certification is preferred.

3. At least one year experience as a Service Coordinator/Case Manager is preferred.

4. Traveling is a requirement for this position. Possession of a valid driver’s license, as well as a licensed and insured automobile is required; or other means of reliable transportation immediately available to individual.

5. Basic office equipment used: computer, printers, telephone, copy machine, fax machine and calculator is required, as is operational familiarity with Microsoft Word and Excel software and TCM software.

About seventy-five percent (75%) of time is spent sitting, talking and hearing. About fifty percent (50%) of time near and far vision are utilized. About twenty-five percent (25%) of time is walking, reaching, low intensity reaching, low to medium intensity fingering. About ten percent (10%) of time is standing, climbing stairs, low intensity carrying, or being outdoors in cold weather. In unusual or non routine situations there could be stooping, kneeling, crouching, bending/twisting, low intensity pushing/pulling, or exposure to hazards of fast moving vehicles or physical attack or injury from clientele.
