Social Worker - Edgewater Haven

Purpose of Position
The purpose of this position is to complete routine pre-admission and discharge duties. Assist the Social Services Coordinator with required assessments and follow up. Assist with public and community relations activities. The work is performed under the supervision of the Social Services Coordinator.
Essential Duties and Responsibilities
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
Conduct pre-admission interviews with referral sources, residents and/or family members where appropriate and complete admission documentation.
Responsible for documentation of observations regarding mood, behavior, socialization, family contacts, staff contacts and other pertinent psycho-social needs.
Provide information and support to residents and families as needed.
Complete Minimum Data Set (MDS) interviews and related documentation as needed.
Assist with promotion of Edgewater Haven Nursing Home and community relations.
Provide discharge coordination services and referrals to ensure smooth transition to setting of choice.
Assist in Level 1 Screening and follow up as needed.
Make appointments and arranges transportation for residents.
In the absence of the Social Services Coordinator, the Social Services Assistant may assist with developing care plan goals and approaches, attend and participate in care plan conferences, complete and enter MDS material, and document quarterly progress notes with direction from MDS Coordinator and Nursing Supervisor.
*Report all resident complaints and suspected abuse of resident’s rights to Social Services Coordinator or Director of Nursing and Administrator or their designee.
*Adhere to Federal and State regulations and guidelines, including Resident Rights and Confidentiality.
*Responsible to know and practice the Safety policies of the County. Perform all job tasks in a safe and prescribed manner.
*General responsibilities of all Edgewater Haven Nursing Home employees.
Additional Tasks and Responsibilities
While the following tasks are necessary for the work of the unit, they are not an essential part of the purpose of this position and may also be performed by other unit members.
Assist with resident’s adjustment to the facility.
Instruct staff regarding resident rights and follow-up to see that those rights are maintained.
Greets and directs visitors.
Answers telephone calls and relays messages.
Escorts residents throughout the facility.
Minimum Training and Experience Required to Perform Essential Job Functions
Bachelor’s Degree in Social Work, one to two years social work experience preferably in a long-term care facility or working with geriatriac patients, or any combination of education and experience that provides equivalent knowledge, skills and abilities. Possession of Wisconsin Social Work Certification is desired. Basic everyday living skills, the ability to understand and follow directions (agency manuals, etc.), and reading and writing (for reports) is necessary. Ability to communicate orally and in writing with individuals and groups. This position may require travel to and from off-campus sites. Individual may use personal vehicle or other means of reliable transportation.
