The Wood County Human Services Department, Family Services Division, has an immediate opening for a Support & Service Coordinator. 


One of the primary purposes of this position is to provide needed support and services to functionally eligible children who have severe physical, cognitive, and emotional disabilities; that are the responsibility of Wood County.  Coordinator will also support the immediate family members of these children and help problem solve issues/concerns that arise.  This position has an impact on the children and families served, as well as on the organization, as it allows Wood County children to receive the items and services they are in need of, to keep them safe and allow them to stay within their home and community, (least restrictive and most natural environment) rather than having them placed in more restrictive and less natural environments.


The following duties are normal for this classification.  These are not to be construed as exclusive or all-inclusive.  Other duties may be required and assigned.
-Complete paperwork other than Functional Screens (case notes, letters, emails, faxes, funding requests/authorizations, WPS authorizations, Individual Service Plans (ISP), Outcomes, referrals, reading over medical records, reviewing Individual Education Plans (IEP), reviewing parents/guardians requests of items/services, completing incident reports through Citr, entering data into the ForwardHealth Portal using Eligilbity and Enrollment Streamlining (EES), high cost funding requests, parental fee calculations, other state and county forms, assessments, etc.).
-Make home visits to conduct intakes, reviews, check-ins, provide support, attend school meetings and/or medical appointments, etc.
-Complete Functional Screens (initial, annual, updates).  Update Children’s Functional Screens annually.
-Make telephone calls to clients, providers, state contacts, therapists, evaluators, contractors, and other professionals and non-professionals associates with the provisions of the County operated services. 
-Conduct assessments and develop and implement ISP’s for children in the Children’s Community Options Program (CCOP) and the Children’s Long-Term Support (CLTS) Medicaid Waivers Program.
-Create and help manage CLTS Waiver Third Party Administration TPA authorizations with the Business office staff. 
-Provide direct supportive services to children and families on the caseload including information/referral, supportive counseling, assistance in making application for financial assistance programs, etc.  
-Perform all duties in compliance with the Medicaid Home and Community-Based Services (HCBS) Waiver Manuel for the CLTS Waiver Program and standards set out in Medicaid Targeted Case Management regulations.  
-Manage the expenditures of CCOP funds, submit timely vouchers, and work closely with the Business office staff in completing State required Reporting Systems (Human Services Reporting System and Program Participation System).
-Attend home visits and discuss family issues/concerns and document all relevant information within CCOP and CLTS Medicaid Waivers Program.
-Responsible to know and practice the Safety policies of Wood County.  Perform all job tasks in a safe and prescribed manner.

-Research items and services that clients are in need of.
-Be familiar with and follow established Wood County Human Services Department policies and procedures, as well as be compliant with federal and state policies and standards. 
-Participate in unit meetings and attend training/conferences as required. 
-Assure that proper case files are maintained in compliance with Department policy and any applicable regulations.
-Monitor service costs for clients in CLTS and CCOP.
-Any other duties as may be assigned.
No supervisory responsibility.
Job Qualifications:

-A Bachelor’s of Arts/Bachelor’s of Science degree in a health or human services related field and at least one (1) year of professional experience working with individuals and/or children with disabilities required.  Must have knowledge working with children with DD (Developmental Disabilities), SED (Severe Emotional Disturbances), and PD (Physical Disabilities).  
-Must be knowledgeable of the service delivery system, the availability of integrated services and resources, or the need for such services and resources to be developed.
-Must complete the complete the approved DHS waiver basics training course and the Mandated Reporter Online Training.  Until training is completed, the Coordinator must work under the direct supervision of a qualified Support Service Coordinator and/or supervisor.  
-Must become certified to perform the Children’s Long Term Support Functional Screen within one (1) month of employment.  

Candidates must possess basic everyday living skills, the ability to understand and follow directions, the ability to read and write (for reports); and the ability to interview, counsel and communicate.  This position requires travel to and from off-campus sites.  Possession of a valid driver’s license, as well as a licensed and insured automobile is required; or other means of reliable transportation immediately available to individual.  

Basic office equipment used:  computer/tablet, printers, telephone, copy machine, fax machine and calculator is required, as is operational familiarity with Microsoft Word and Excel software and The Clinic Manager (TCM) software.
About seventy-five percent (75%) of time is spent sitting, talking and hearing.  About fifty percent (50%) of time near and far vision are utilized.  About twenty-five percent (25%) of time is walking, reaching, low intensity reaching, low to medium intensity fingering.  About ten percent (10%) of time is standing, climbing stairs, low intensity carrying, or being outdoors in cold weather.  In unusual or non routine situations there could be stooping, kneeling, crouching, bending/twisting, low intensity pushing/pulling, or exposure to hazards of fast moving vehicles or physical attack or injury from clientele.
